McDonald's is one of the most-searched restaurants for diabetics and people on low-carb diets — and with a few swaps, you can build a meal under 20g net carbs without feeling deprived. Here's a working guide to the menu, sorted by what to order, what to skip, and how to think about glycemic impact (not just total carbs).

Why glycemic load matters more than calories

The carb numbers on a McDonald's bun, fries, or McFlurry land in your bloodstream fast — they're refined flour, refined potato, and added sugar. A 50g carb burger bun spikes glucose harder than 50g of carbs from beans. The smart-swap rule is simple: trade bread, breading, and sugary sauces for protein, lettuce, cheese, and fats.

The headline numbers (net carbs)

Approximate net carbs for popular items, based on McDonald's US nutrition data:

  • 10-piece Chicken McNuggets: ~22g
  • 6-piece Chicken McNuggets: ~13g
  • Quarter Pounder with Cheese: ~40g (~36g from the bun)
  • Big Mac: ~44g
  • McDouble: ~32g
  • McChicken: ~38g
  • Egg McMuffin: ~29g
  • Sausage McMuffin (no egg): ~28g
  • Medium fries: ~43g
  • Small fries: ~25g
  • Apple slices: ~8g
  • Side salad (no dressing): ~3g
  • Sweet 'N Sour sauce (1 packet): ~10g
  • Tangy BBQ sauce (1 packet): ~10g
  • Hot Mustard: ~7g
  • Spicy Buffalo: ~1g
  • Creamy Ranch: ~2g

The single highest-leverage swap on the entire menu is the bun and the dipping sauce.

The "no-bun" trick

Almost every McDonald's burger can be ordered "no bun" — they'll wrap the patty, cheese, pickles, onion, mustard, and ketchup in lettuce or just paper. Carbs collapse from ~40g to ~5-8g:

  • Quarter Pounder no bun: ~8g net carbs
  • McDouble no bun: ~6g net carbs
  • Double Cheeseburger no bun: ~7g net carbs

Ketchup adds ~2g per packet — skip or use mustard (almost zero carbs).

Better nugget strategy

A 10-piece is the most-searched McDonald's order in the US, so it's worth optimizing. The breading carries ~2g of carbs per nugget. Cut the order to 6 pieces (~13g) and pair with Spicy Buffalo (1g) or Creamy Ranch (2g) instead of Sweet 'N Sour (10g) or BBQ (10g). You save another ~9g without changing the nuggets.

If you want more food, add a side salad (3g) or apple slices (8g) before adding fries.

Breakfast that won't spike you

  • **Round Egg + Sausage + Cheese, no muffin**: ~2g net carbs. McDonald's calls this "Folded Egg" or just ask for "the inside of an Egg McMuffin." Most stores will plate it for you.
  • **Sausage Burrito with no salsa**: ~26g — borderline; skip the hash brown.
  • **Hash brown**: ~14g and a huge glucose spike. Replace with apple slices (8g) or skip.
  • Coffee, black or with sugar-free vanilla: 0g. Stay away from the lattes — a medium latte is ~17g.

Sides and drinks

  • Replace medium fries (43g) with a side salad (3g) for a ~40g savings.
  • Diet Coke, unsweetened iced tea, sparkling water: 0g.
  • A medium Coke is 65g — equivalent to 16 sugar cubes. Worth knowing before you order it on autopilot.

A sample sub-20g order

A real meal you can order today:

  • 6-piece Chicken McNuggets (~13g)
  • Spicy Buffalo sauce (~1g)
  • Side salad, no dressing (~3g)
  • Diet Coke (0g)

Total: ~17g net carbs, about 30g protein, satisfying enough for lunch.

A burger lover's sub-15g order

  • Quarter Pounder with Cheese, no bun (~8g)
  • Side salad (~3g)
  • Add bacon (~0g)
  • Black coffee (0g)

Total: ~11g net carbs, around 35g protein.

Things to skip almost every time

  • Buns and tortillas — refined wheat, fast spike.
  • Fries and hash browns — refined potato, fast spike.
  • Sweet sauces — BBQ, Sweet 'N Sour, Sweet Chili, ketchup in volume.
  • McCafé flavored drinks — usually 30-60g of added sugar.
  • McFlurries, shakes, and pies — dessert spikes are the hardest to recover from.

How Carb Lens helps mid-meal

If you forget the numbers, snap your tray with the Carb Lens scanner — the AI recognises McDonald's items by shape (a McNugget, a small fries, a Quarter Pounder bun) and returns a per-item net-carb breakdown in about a second. It also flags the highest-impact item on the tray and suggests a swap. For drive-thru orders it doubles as a sanity check before the first bite.
